Zonneveld Osteopathy is a practice for osteopathy located in Amsterdam East. The practice is located in the characteristic old hospital. Osteopath Kai Zonneveld D.O. MRO® is linked to practice. On February 12, 2017, Kai obtained the title of Diploma in Osteopathy (D.O.) with scientific research into the effectiveness of osteopathy in irritable bowel syndrome.

Zonneveld Osteopathy is registered with the Dutch Register of Osteopathy (NRO) under registration number 10123.

Opening hours

Monday to Friday from 08:00 to 17:00. If you want to make an appointment outside the specified time, this is possible, please contact the practice.

Cancellation

If you are unable to attend, please contact the practice at least 24 hours in advance. This way you give other people the opportunity to schedule an appointment in the short term. Without timely cancellation, 50% of the rate of the intended treatment will be charged.

prices

An osteopathy consultation costs € 95. An osteopathy consultation for babies and children costs € 70. You can pay the consultation by pin and / or cash.

Insurance & compensation

Most health insurers fully or partially reimburse the treatment from the supplementary insurance. Your deductible is not taxed. It is best to consult your policy or insurer for correct information. Zonneveld Osteopathy is registered with the Dutch Register of Osteopathy (NRO) under registration number 10123.

The treatment

The first treatment consists of an intake, examination and treatment. It will be discussed with you whether your complaint is an indication for osteopathy. A treatment plan is drawn up on the basis of the results of this study. This treatment plan includes the form of therapy. The treatment takes about 45 minutes.

You may have to wait a little longer. Treatment may be delayed due to circumstances. We ask for your understanding.

privacy

The practice has closed treatment rooms, which guarantees your privacy.

The osteopath keeps a record of your medical and administrative data in order to perform your treatment as well as possible. The Personal Data Protection Act applies to this registration. The osteopath handles your data carefully. The osteopath may not pass on data to third parties without your permission.


complaints

Your osteopath would of course like to hear it if you are satisfied. Unfortunately, it can also happen that your treatment does not go completely as desired. Then it is pleasant for both you and your osteopath to discuss this with each other and to come to a solution.

Always discuss any complaints as openly as possible with your osteopath. Your osteopath can only respond to this when you make your complaints known. In the vast majority of cases, a good conversation leads to a satisfactory solution.

If you are unable to resolve the matter jointly, or with the mediation of the complaints officer, you can submit your complaint to the independent Disputes Committee for Osteopathy.

Procedure General Data Protection Regulation (GDPR) 07-04-2018

This procedure describes how the requirements of the GDPR are met within Zonneveld Osteopathy. This law applies from 25 May 2018.

Controller responsible:
Within practice, there is only one controller (the owner) and no other processors of personal data.

Recording personal data:

There are two situations within the company where personal data is recorded:

1. For the benefit of the administration.

This concerns: name, address, telephone number, email and date of birth.

Purpose: This data is used for sending the invoice to the patient by email. Medium: the osteopathy software package Crossuite and accounting.

2. For the benefit of the medical file.

The medical file contains the special category of personal data, according to the GDPR. The processing of this special category of personal data is not prohibited for healthcare providers, where the processing is necessary for the proper treatment of the data subject or the management of the professional practice. Firstly, this concerns the information as requested in the intake form and concerns data about the patient's health. This information is provided by the patient himself. Secondly, the file contains the consultation reports.

Purpose: To provide good treatment, whereby the preparation of the research and treatment plan and the adjustment thereof. Under the Medical Treatment Contracts Act (WGBO), care providers are obliged to keep a medical file. Means: intake form and consultation reports, stored in the osteopathy software package Crossuite.

Sharing with third parties: The medical file is not shared with third parties (general practitioner, health insurer, etc.) unless the patient has given permission.

Retention period for medical records: Practice is required to keep patients' medical records for fifteen years, "or as much longer as the care provided by good care services". For minors, the retention period starts from the eighteenth year of life. Medical records of deceased patients must be kept for fifteen years after the last change in the file.

Patient privacy rights: The patient's privacy rights can be found on the website under the Privacy Statement and in the appointment confirmation.

Privacy statement: Before your first consultation, you will be asked to fill in an intake form, in which you will also state your complaint and medical history in addition to your name and address details. We discuss this information during the initial consultation and help draw up the treatment plan. Your data is covered by professional secrecy and will not be provided to other third parties (general practitioner, health insurer, etc.) unless you have given permission. Your medical file is kept for 15 years in accordance with the law. You have the right to inspect your medical file and the right to request correction or deletion of your personal data.
